Forms: All campers must submit a Health Record and a Liability Waver Form in order to attend camp. Campers may use QSC's Health Record Form or submit a CT School Medical Form signed by a physician in the recient 36 months. Submission of the Medical Form is a CT State Requirement to attend camp.
